New Selian Children Hospital Arusha Tanzania

In 2015 James Buy Sell helped set up a business plan with Sarah Rejman for Plaster House Children’s Rehabilitation in Arusha Tanzania

Whilst the massive improvements for the came largely from Sarah’s and other staff members incredibly hard work in the complete revitalisation of The Plaster House – it also came from that 2015 Business Plan. The 2015 Business Plan gave Sarah and her dynamic team a roadmap to follow to achieve excellence. You can see a copy at the bottom of this post.

James Buy Sell now has a three-year funding agreement in place with The Plaster House and whilst we don’t use this funding site anymore (use above site at Plaster House website is direct and 100% gets there) – this site tells a good story about Sarah’s work and James Buy Sell connection – yes we climbed Mt Kilimanjaro. The New Selian Childrens Hospital

What we are trying to do at James Buy Sell and youcanchangealife is create a similarly dynamic business plan, to implement for a hospital that has been ravaged by poor financial and other management in the past from previous directors.

Selian Hospital is very near Plaster House in Arusha Tanzania and is used by Plaster House for some child surgeries.

We have been twice before and the conditions are best described as basic.

We are currently supporting Selian cover costs for Child Surgeries – we are doing that through James money on which we pay tax first because we don’t believe the financial systems are robust enough yet to go through an Australian charity system and get deductions. We are doing this to enable us to develop a business plan for supporting the poor children who need surgery from the villages who will be brought to SelIan Lutheran Hospital and seen by specialist, evaluated and have safe surgery and discharged home after fully recovery with good health. Part of this business plan also will be to enable one person to go to the villages to give education and collect all children who need surgery. Also part of it will be to renovate and make a good environment for the children to be in the wards and playing environment. Also to have the attendants who wiill take care of them while in Selian during the recovery for surgery. This business plan will include also the plans for feeding for these children while in the hospital. The business plan will show the relationship and partnership between the supportors who are YOUCANCHANGEALIFE and the implimenter Selain Lutheran Hospital.

In Arusha region ALMC is one specialised for children but most of the one getting treatment are those with national health insurance

  Yes, we are ready to specialize and We will be happy to be the excellent center for pediatric/children surgery center.

7.What are Selian’s most common children medical operations

Hernia repair, laparotomy for intestinal obstruction, tonsillectomy, nasal polyps cauterization, burn contracture release, cleft lip and palate repair

Pre operation care – doctor review, anathetics review, nurses revies

–        vital sign

–        bath and preparatory for surgical site

–        filling concert form

–        Family conseling

–        Putting a tag for name and file number

Operational procedure – received operating room, verification of name and file number and planed procedure, final anesthetic review in the operating room, given anesthesia, doctors scrub and cleaning surgical site and draping done. Then surgery is done.

Post op care – start in thieter after operation in recovery room, then after recover from anesthesia tranfered to the ward. The monitoring include vital sign, given planed postop medication, care of the wound site. In the evening review again with a doctor in the ward and next day majour ward round review.

I read,  I like the idea. This is a good plan for Selian Hospital and the support for the children who need surgery. My suggestion is to improve the physical and occupational therapy and provide important equipment. This is a crucial part for healing and reduces disability to the operated children.

Administration

1.Who works in administration (non medical and non finance) and what are their roles please? Eg cooks , cleaners and ………

In admininistartion we have the hospital director overall incharge, Director of clinical services supervising all hospital clinical services, Hospital health secretary responsible for all human resource issues and maintenence of building and machines and medical equipment. Head nurse supervising all nursing activities and all cleaners for the hospital and chief accoutant responsible for financial issues. We don’t have cookers.

2.Do you have any administration systems?

 Yes, we do.

3.If yes to any systems please describe them?

I can share with you organogram.

4.When a child patient comes to Selian what happens?

The child will be received at out patient department, if comes, as an emergency will be seen at causality and admitted. Those who are cold cases will be seen by general practioner and assess them and send them to specialist clinic. They will do primary investigations. When they see the specialist at clinic will be evaluated more and more investigations. If they need surgery will be admitted for surgery the next day or the other day. Those admitted will be seen with specialist in the ward.

5.Who feeds and baths and cares for them?

For serious children feeding  and baths is done by nurses and attendants but those who are not serious are fed and bath  by parents mostly mothers  or relatives who stay with children.  Currently the food is brought from home OR parents can buy from the hospital canteen but those who wants to cook kitchen is available for them to cook.

6.Who cooks their food?

  Currently the food is brought from home OR parents can buy from the hospital canteen but those who wants to cook kitchen is available for them to cook.

7.Where do their parents stay?

We allow children to stay with their parent mostly mother in the hospital, they share the bed.

8.How long is the average child staying at Selian?

5 to 7 days

9.How does administration communicate with doctors and finance please?

We have meeting for management team, management committee and doctors meeting. Finance are welcomed to attend this meetings. 

10.What are the 3 biggest improvements you can suggest for Selian

  1. a) Improving wards b) improving staff salaries and allowances c) availability of medicine, medical supplies and laboratory reagents

Yes I read, it is a good plan for Selian.

My suggestions are to complete the write-up and include other care including physiotherapy, occupational therapy and feeding for children

My focus with you and Amon and Remy is to get a system working over 2020 more so than rush headlong into lots of things.
I pay for surgeries out of my own money and I have to pay tax on them.
If we are going to go big we will need funding and we need to have good systems to appeal to funders.
I actually want to set some money aside for developing these systems unless I can find donors.

This is bringing us more close and understanding each other.

I think we need to agree together on supporting these children.

  1. Number of surgeries per month? depend on Letion outreach and conditions
  2. Total cost per month, how much youcanchangealife can pay per month? i think you can help us on this. Just give us a figure and we will not go beyond that.
  3. Selian to give food to children or not. Usually we dont give but we are in position to get the food to the children if we agree and we can agree the cost of food per day. It may cost around 10,000 TSH per child per day and not more than 20,000 TSH for the child and the mother.

I agree with you we need to improve the environment for these children to have safe surgery and safe environment.

I suggest, we can start with painting and minor renovations for surgical ward and operating theater. Can we prepare a small project on this? can we make some estimation?

We still we can give a good postop care at Selian by retaining these Children and discharged when a fully recovered to go home.

We have playgrounds at Selian with some children meet twice per month, we have a club for children who are orphanage and those from difficult environment and HIV children meet on saturday club. We are able to expand these grounds and create more sports for children.

I think Letion can do good outreach and us at Selian we are following closely to make sure he dont take extra money from these poor parents and i agree with you he should be stopped as soon as we note this.

I take this opportunity to bring our and gratitude for the support of these poor Tanzania children for surgery. These generous support has changed their life and their family. May God bless you all.

Letion didnt deposit or pay the 2,000 USD to Selian, he used for food. I hope you will communicate with him for that. He will send the photos for children attended in September and October. Next week i will send you October invoice and the report. Do you have any suggestions to improve the reports? Remy is preparing an excel report on finance to show the invoices we send to you and the amount received from you. We will share with you as soon as possible.

I am very happy to hear your plans to visit Arusha in January 2020. Will be our pleasure to meet you again and have more talk and plans to help these poor children who need surgery. It will be a good time also to have long term plans for this program.

  1. Do each of you as medical doctors and medical staff want to be become well paid? The answer is YES

We will be happy to be paid well. The most important is the staff to get their rights as medical personel on salaries, allowances for extrawork, night shift and be paid in time.

  1. Do you want to provide a lot of surgery to children who are poor? The answer is YES

The vision and mission of the hospital is to serve the community and the poor children are part of the community and these are under served population who need a help and Selian getting more funds to support these kids will be our priority and we will be happy to have more children getting surgeries in Selian.

  1. Do you want to work in good hospital?  The answer is YES

We always dreaming and working hard to work in good environment. Improving our infrastructure and equipment is of highly important for the good hospital. The safety of surgery is guaranteed in a good hospital. We will be happy to have more equipment and renovating and extending our wards to get a good hospital. Staff motivation also is part of good hospital.

  1. How do you think we can help you at Selian be paid well? You can help us by 4 ways
  2. i) supporting surgeries for poor children, this gives income to the hospital and i can suggest even in future Selian to offer eye surgeries to Children. We can open eye clinic and help to do children eye surgeries and reduce blindness in the community. This is under served part in Northern Tanzania.
  3. ii) Support for medical equipment and medical supplies to the hospital

iii) Assist renovation and extension of our wards and improving the hospital environment

  1. iv) Directly support for salaries or top up for salaries or allowances to our staff at Selian
  2. How do you think we can help you treat more poor children at your hospital?  You can help us by supporting the community outreach either through Letion or Selian team to go to the community by doing outreach and get more kids from the villages who need surgery and missed that opportunity, these will help to get more children for surgery. You can help us also to do renovation and extension of surgical ward and get a space for rehabilitation of these children before discharged home. The other way is to have a kitchen and provide food to these children and have more nurses and assistant nurses to care for these children.
  3. How do you think we can help you make Selian a centre of good care? You can help by supporting equipment such as CT SCAN, C-ARM, other theater equipment and supplies, having oxygen plant and better ICU with good monitors and ventilators and improving our wards and assisting the hospital to provide food for the operated children and other poor patients.

Selian:  Do you have any suggestions to improve the reports?

youcanchangealife: Yes we have a number of suggestions, but the main thing right now is we don’t really communicate as good as we could and we need to work on that together.

We want to understand and agree what our $ commitments are to Selian and Selian can agree and understand what your medical care commitments are to youcanchangealife.

For example nobody has asked us what we can afford each month and is it ok to buy extra food (can we afford that?) and would we like / can we afford to pay for 2 or 5 or 20 surgeries a month?

We started off with Letion in Feb 2019 and agreed to a small monthly payment to him and not much else

Having said that – we like you all and other people at Selian and the reports are a very good start – we think you are doing a very good job, we want to support you. We will need to work together on some budgets in 2020

This is how we at youcanchanegalife look at your Selian reports. The pink are ones we cannot fund as there is no record from Letion.  We have a written agreement with Letion, he only tells patients he is with you at Selian and he takes NO MONEY from poor people needing surgery. If he takes money – we will stop working together.

The vision and mission of the hospital is to serve the community and the poor children are part of the community and these are under served population who need a help and Selian getting more funds to support these kids will be our priority and we will be happy to have more children getting surgeries in Selian.

Ok we can show you how to present to donors – its hard to get new donors and easy to lose them when you have them. If you communicate, are consistent and your care is good, then you will get donors.

  1. Do you want to work in good hospital?  The answer is YES

We always dreaming and working hard to work in good environment. Improving our infrastructure and equipment is of highly important for the good hospital. The safety of surgery is guaranteed in a good hospital. We will be happy to have more equipment and renovating and extending our wards to get a good hospital. Staff motivation also is part of good hospital.

These are good thoughts – the key is CLEAR GOALS and GOOD SYSTEMS with GOOD MOTIVATED DOCTORS AND STAFF and keep improving. You are thinking well Amon, as is Elibariki.

  1. How do you think we can help you at Selian be paid well? You can help us by 4 ways
    i) supporting surgeries for poor children, this gives income to the hospital and i can suggest even in future Selian to offer eye surgeries to Children. We can open eye clinic and help to do children eye surgeries and reduce blindness in the community. This is under served part in Northern Tanzania.

OK, but we need to make sure Selian is good at basics first eg broken bones and tonsils and that people trust our medical care and trust Selian accounting and then Selian can look at eye clinic donors. But this sounds a really good idea Amon.

ii) Support for medical equipment and medical supplies to the hospital

Yes, but we need to get good systems with what we have and we need to have good security to prevent theft and then Selian will more easily find a donor willing to buy expensive equipment.

iii) Assist renovation and extension of our wards and improving the hospital environment 

We agree – but let us start with one ward first – a children’s ward – lets get one ward working well and then raise money off the success and then spread to other Selian wards.

iv) Directly support for salaries or top up for salaries or allowances to our staff at Selian

If you can have good systems that account for money and patient care and equipment then donors would more easily support your salaries. We need to get rid of The Old Selian and bring in The New Selian that everybody can trust.

  1. How do you think we can help you treat more poor children at your hospital?  You can help us by supporting the community outreach either through Letion or Selian team to go to the community by doing outreach and get more kids from the villages who need surgery and missed that opportunity, these will help to get more children for surgery. You can help us also to do renovation and extension of surgical ward and get a space for rehabilitation of these children before discharged home. The other way is to have a kitchen and provide food to these children and have more nurses and assistant nurses to care for these children.

This is a good plan – I think this we can help sponsor – meaning Good Outreach and then good Post Op care at Selian and Good Food and Sports – we love children to play and get soccer balls all the time – they are so happy when that happens. Selian needs to get our before and after care as good as our surgery.

  1. How do you think we can help you make Selian a centre of good care? You can help by supporting equipment such as CT SCAN, C-ARM, other theater equipment and supplies, having oxygen plant and better ICU with good monitors and ventilators and improving our wards and assisting the hospital to provide food for the operated children and other poor patients.

Ok we agree on equipment – so lets get really good systems going first and then donors will support Selian with equipment when they trust again.
